Student Transcript Request
McDade ISD is happy to assist you with obtaining a copy of a student transcript. To request a transcript (printed or sent electronically to a college or institution), please complete and submit the form below.
All required information is necessary to verify the identity of the individual requesting the transcript and to ensure records are released only to authorized parties.
If the student is under 18 years of age, a parent or legal guardian may submit the request.
If the student is 18 years of age or older, the request must be submitted by the student unless a valid Power of Attorney (POA) is provided.
For in-person pickup, a valid photo ID will be required at the time the transcript is released.
